To monitor and measure the most important outcomes that emerged from the new Patient Pathway and the related services implemented.

YBP leverages digital platforms that allow to:

  • Remotely monitor the patient's health in a tailor-made way that is easy to modify for the clinician.

 

  • Connect all key stakeholders around the patient to ensure a support network ready to respond to

        the person's needs.

 

  • Make the patient journey efficient by minimizing waiting times and optimizing costs.

REAL CASE

We simulate cost optimization by assessing the investment required to enhance the patient pathway

and comparing it to the potential savings resulting from reduced patient complications and subsequent

re-hospitalizations or treatments.

Through this analysis, we can estimate the value of the newly designed services or strategies.
